What to Expect
You will be greeted by a receptionist that will take your insurance and demographic information. Your therapist will meet with you to obtain your medical history, concerns, and work with you to build a plan of action to help you meet your goals.
Necessary Paperwork
We know nobody likes filling out paperwork, but we also know that nobody likes to wait longer to get started. Feel free to download, print off, and fill out some of your paperwork before you come in to save some time.

Tips for your first visit
• Arrive 10-15 minutes prior to your first appointment.
• Wear or bring loose fitting clothing that is easy to move around in (for physical therapy evaluations or pediatrics)
• Bring your insurance card, prescription, referral, and all correspondence if you have a Workers' Compensation or MVA Claim. For pediatric patients, the child's IEP is also helpful if available.
• Please feel free to complete your paperwork beforehand: A list of necessary paperwork is below. Please remember to bring all completed forms to your first appointment.
